背景介绍：
皮质醇是最丰富的循环类固醇，也是肾上腺皮质分泌的主要糖皮质激素。皮质醇是生理上有效的血压维持和抗fl炎症活动。它还参与钙的吸收、糖异生以及胃酸和胃蛋白酶的分泌。
血皮质醇水平的测定可作为肾上腺功能的指标，对阿狄森氏病、库欣病、肾上腺增生及癌的鉴别诊断具有重要意义。
大多数循环中的皮质醇与皮质醇结合球蛋白或反式皮质醇结合。因此，尿液中游离皮质醇的浓度很低，24小时的尿液采集是获得准确测定尿皮质醇的必要条件。血液中的皮质醇呈昼夜节律，早晨含量最高，夜间含量最低。
Cortisol is the most abundant circulating steroid and the major glucocorticoid secreted by the adrenal cortex. Cortisol is physiologically effective in blood pressure maintenance and anti-infl ammatory activity. It is also involved in calcium absorption, gluconeogenesis as well as the secretion of gastric acid and pepsin. Measurement of blood cortisol levels can be used as an indicator of adrenal function and the differential diagnosis of Addison’s and Cushing’s diseases as well as adrenal hyperplasia and carcinoma. Most circulating cortisol is bound to cortisol binding globulin or transcortin. Therefore, the free cortisol concentration excreted in the urine is very small, and the 24-hour collection of urine is a must in order to obtain an accurate measurement of urinary cortisol. Cortisol in blood shows a diurnal rhythm with the highest levels in the morning and the lowest levels at night.
